Who is a good fit

  • A veteran, 18 or older
  • Experiencing or at imminent risk of homelessness
  • Living with a disability
  • Able to live independently in their own apartment

Our response commitment

We respond to every referral within two business days. If our program isn't the right fit, we'll tell you promptly and, where we can, suggest another option.

Do not submit client PII through this form. Use the veteran's initials only — no full names, Social Security numbers, dates of birth, or medical details. We'll gather what's needed securely after we connect with you.
